  • BSA Medical Forms A/B – This form is required for all scouting activities less than 72 hours.
  • BSA Medical Forms A/B/C – This form is required for all scouting activities longer than 72 hours. Most overnight summer camps will require this form.

Medical forms are required by Scouting America for both the cub scout and the parent attending the event that triggers the requirement.

You may hand your form in directly to the Committee Chair at any Pack meeting, or deliver it to the person in charge of medical forms on any given overnight where it is required.

At your option, you may also e-mail the form to the pack email address: pack1592@gmail.com and we will print one copy to put in the medical binder, and then destroy the email to protect your information.

If you give us the form directly, we will not copy, digitize or share the form. It will be held on file for one year for use in medical emergencies as required by Scouting America. After one year the form is no longer valid and it will be destroyed.
